Tandem Expands to Support Coordinated Access for Live-in Treatment
 
As of April 1, our 24/7 crisis support and mental wellness program, Tandem, is expanding to support coordinated access for live-in treatment in London-Middlesex, Huron Perth, and Oxford Elgin.
 
This means families can begin their journey through one central entry point, sharing their story once and connecting to the supports that best meet their needs.

Tandem Live-in Treatment - Our New Process 

All referrals for live-in treatment and CPRI intensive services will now follow our coordinated access shared pathway. Here’s how it works:

  • You can make a referral using this link - https://forms.office.com/r/AzcxEsBw4R
  • You’ll need to fill out the intake form, and then the Tandem team will contact you about eligibility for live-in treatment and next steps.

  • If you have feedback about the system or process, please submit it here: mglab [at] vanier.com (mglab[at]vanier[dot]com). Your input will be incredibly valuable as we all adjust to this new system across our region.

How we help

  • Crisis Response and 24/7 telephone support with brief follow-up counselling in urgent situations.
  • The Tandem team coordinates the intake process for Vanier, Merrymount, and Humana.

Who is eligible?

Children and Youth from birth to 18 years of age and their parents/guardians.
