ACT (Assertive Community Treatment Team)

Assertive Community Treatment (ACT) is an intensive treatment program developed for children and

youth ages 8-17 who have been identified as having significant mental health needs and have not been successful in engaging with traditional mental health services. The ACT Team utilizes a flexible, multidisciplinary approach composed of Nursing, Child and Family Therapists, Occupational Therapy, and Recreational Therapy to develop and implement treatment plans tailored to each client's individual needs. The program’s primary goal is to empower children and youth to engage actively in community-based services, enabling them to remain safely and successfully in their homes, schools, and communities.

Clients may be referred through Tandem via self-referrals, community agencies, hospitals, primary care providers, educational institutions, and other sources.
